Distributors can type their ARN before filling in the other details. Investors opting for the 'Direct Plan', should type the word 'Direct' in the box meant for the Distributor Code. Page 27 contains a grid which mentions the relevant documentation to be attached alongwith the Form. In case you are submitting both the applications (Lumpsum + SIP) concurrently, you need not attach two separate cheques, as long as you are using the same bank account for both the transactions. You may deposit this Form either at your nearest CAMS Investor Service Centre or send it to any of our branches. Please do not courier your Application Form to CAMS.

Filling and submitting this 'One Time Mandate' Form will give you the flexibility regarding the amount that you wish to invest in our scheme.
You can authorise the bank to accept any investment instruction upto a pre-decided maximum amount. Investments can currently be undertaken offline or online.
